چکیده

T raumatic extracranial carotid lesions can result from blunt, penetrating, or iatrogenic injuries. Blunt cerebrovascular injuries are found in up to 0.1% of all patients hospitalized for trauma in the United States, but the incidence can be as high as 2.7% in patients with an Injury Severity Score > 16.1 Of note, blunt lesions are associated with neurologic morbidity up to 80% and mortality as high as 40%. Penetrating injuries to the carotid artery occur in up to 10% of all cases of penetrating neck trauma2 and are also associated with a high mortality rate (50%).3 Finally, iatrogenic carotid injuries, in most cases, are associated with misplacement of central venous lines with an incidence of 0.1% to 0.8%.4 Management of extracranial carotid lesions varies from medical therapy to endovascular or open repair, depending on the severity of the lesion and the patient’s overall clinical status. Endovascular repair has gained more acceptance given its decreased invasiveness, safety, and effectiveness.

Endovascular repair of traumatic cervical internal carotid artery injuries: a safe and effective treatment option.

BACKGROUND AND PURPOSE The appropriate choice of treatment for traumatic extracranial carotid artery injury is still debated. The purpose of this study was to evaluate outcomes of endovascular carotid repair with regard to vessel patency and retreatment rates. METHODS AND METHODS We retrospectively reviewed records of patients who underwent endovascular treatment for acute traumatic internal ...

The role of CT angiography in the diagnosis of blunt traumatic thoracic aortic disruption and unsuspected carotid artery injury.

We have replaced aortography and open thoracic surgery to diagnose and treat blunt traumatic thoracic aortic disruption (TTAD) in favor of CT angiography (CTA) and endovascular repair. The purpose of this study is to review our experience with the management and outcomes of TTAD and associated carotid artery injuries. In January 2003, we initiated a protocol that used CTA to evaluate all patien...

Endovascular stent-assisted angioplasty in the management of traumatic internal carotid artery dissections.

BACKGROUND AND PURPOSE The prognosis of traumatic dissection of the internal carotid artery is worse than for spontaneous dissections. Rapid stenting followed by antiplatelet therapy may prevent complications when anticoagulation therapy is not applicable. METHODS Patients with angiographically proven traumatic carotid artery dissection and hemodynamic significant hemispheric hypoperfusion, o...

Carotid artery thrombosis and cerebral infarction after multiple traumas

Carotid artery thrombosis and concomitant brain infarction after blunt trauma are rare conditions. We report a 34-year-old woman with multiple traumas due to pedestrian car accident. At the time of initial visiting, she was somnolent and her Glasgow coma scale (GCS) score was 14. Initial vital signs (V/S) and brain computed tomography (CT) scan were normal. One day after admission to the emerge...

Advances in the endovascular treatment of direct carotid-cavernous fistulas.

Carotid cavernous fistulas (CCFs) are abnormal connections between the carotid artery and the cavernous sinus. They are considered direct when there is a direct connection between the internal carotid artery and the cavernous sinus. These cases are generally traumatic. Direct CCFs are high-flow lesions, possibly related to intracranial bleeding, visual loss, corneal exposure or even fatal epist...
